Regular dental checkups can help find early warning signs of certain health problems, which means you can get the care you need to get healthy. LHP/LER will continue to offer a dental program. The chart below is a brief outline of the plan. Please refer to the summary plan description for complete plan details.

PPO Plan: Receive dental care from the licensed dentist of your choice. You’ll receive the highest level of benefits if you select an in-network dentist who has agreed to provide services at a negotiated rate. If you use an out-of-network dentist, they may bill you for the difference between what the plan pays and what the dentist usually charges.
